NOTE:  Lacey Team HQ members only will be able to View CBP Forms

 

 

To view a form, complete the following steps:

 

1.Select the View CBP Forms link at the left side of the screen.

RESULT:  The View CBP Forms page appears.

 

2.Enter search criteria to view a list of forms and click the Search button.  You can search for a form based on the following criteria:
a.Enter the form Status and Date Range (Entry Date, Estimated Arrival Date, Reviewed Date or Submitted Date) and optionally (Importer Organization, Importer Number, Entry Type, Entry Status, Port of Entry or Mode of Transportation.).
b.Enter the Random Sample Date Range (Entry Date, Estimated Arrival Date or Submitted Date) along with Number of Records or Percentage of Records (If you enter both an Error message will be displayed).  You can click the calendar to select the appropriate dates..
c.Or, enter the Entry Number.
RESULT:  The forms associated with the entered search criteria are displayed with the following information:
Entry Number - the U.S. Customs entry number assigned to the shipment.
Importer Organization - this column is viewable only by Brokers and will display the Importer Organization for whom the form was submitted for.
Status - status of the form once Reviewed (Passed, Not Passed, and Further Review Required).
Submission Date - the date the form was submitted.

 

3.To view a submitted declaration form, complete the following steps:
a.Click the corresponding View button.
RESULT:  The Form Details page appears displaying form information and the Submitted On date and Submitted By person.

   RESULT:  A new window opens and displays the Declaration form.
 

 

4.To add Notes to the form:
a.Type your Note in the New Note text box.

 

5. To update the Reviewed Status select the arrow from the drop-down menu and make your selection:

          a. Further Review Required

          b. Not Passed

        c. Passed

 

6.  Select Failure Reason check box (if applicable):

          a. Species/country mismatch - species is not grown in country or species not permitted from.

        b. Quantity error - incorrectly declared number

        c. SUD error - not supposed to use code for that product or incorrect code.

        d. Description error - does not match product/tariff code.

        e. Value Error - incorrectly declared number.

 

7.   Click Save Review to save the form review.

 

8.   Click Cancel (Top Right or window or bottom left of window) to cancel the form review.
