View Forms for Administration

The Lacey Team can access the View Forms page to retrieve declarations.  They can enter reviewer notes, failure reasons, internal notes, and notes to the applicant on the internal version of the Form Details page.  The Lacey Team can also print copies of declarations or download declaration data in Excel or PDF format using the Manage button.

 

To view a form, complete the following steps:

 

1.Select the View Forms link at the left side of the screen.

RESULT:  The View Forms page appears.

 

2.Enter search criteria to view a list of forms and click the Search button.  You can search for a form based on one of the following criteria:
a.Enter the form Status and Date Range (Create Date, Submitted Date, or Reviewed Date).  Optionally you can select the Importer Organization or Broker.
b.Enter the Replacement Date Range.  You can click the calendar to select the appropriate dates.
c.Enter the Random Sample Date Range along with Number of Records or Percentage of Records (Not both).  You can click the calendar to select the appropriate dates.
d.Enter the Tracking Number.

e. Or, enter the Entry Number.

RESULT:  The information matching the search criteria displays in the List of Forms table.  From the resulting list, you will have the option to review and add notes.  You may also manage the forms.

 

3.To view the form, click the corresponding View button.

RESULT:  The Form Details page appears displaying the submitted declaration information.

a.Under the Reviewer Notes section, you may enter notes regarding this declaration form here. NOTE:  Reviewer notes can only be added to Submitted applications, not Work In Progress applications. These notes will not be accessible by the applicant and can only be accessed and/or edited from this page for authorized USDA users of LAWGS.
b.Select the appropriated status from the Reviewed Status drop-down list (Further Review Required, Not Passed, Passed).
c.Select one of the following Failure Reasons:
Species/country mismatch - species is not grown in country or species not permitted from country
Quantity error - Incorrectly declared number
SUD error - not supposed to use code for that product or incorrect code
Description error - does not match product/tariff code
Value error - incorrectly declared number
d.Enter text as needed under the Reviewer Notes to Applicant section.

 

4.Click the Save Review button when you are finished.

3.To manage the form, click the corresponding Manage button.

RESULT:  The Manage Form page appears displaying the Current Form Status, Tracking Number, and Prepared By information.

 

4.Choose the desired process from the Process drop-down list (Generate Excel or Generate PDF).

 

5.Click the Select button.

RESULT:  A new screen appears with the declaration displayed.
