A form template simplifies creating a new form by reusing existing information, such as consignees and merchandise. You may add, update, or delete form templates at any time.
| 1. | To manage form templates, select the Manage Template Data link at the left side of the screen.
RESULT: The Manage Form Templates page appears displaying a List of Templates. |
NOTE: Brokers must select an Importer Organization to proceed. Brokers do not have their own templates, but can create and update templates owned by the Importer Organizations that they have relationships with.
| 2. | To start a new declaration form based on a template, find the template and click the Start New Form button.
RESULT: The Creation of a Plant and Plant Product Declaration Form page appears and you can complete the Form Step By Step process. |
| 3. | To update a template, click the corresponding Update button.
RESULT: The Template Details page appears. You can enter details to be saved in this template and select Save to save the information and return to the list of templates. |
| 4. | To delete a template, click the corresponding check box under the Delete column and click the Delete button.
RESULT: A confirmation page appears asking "Are you sure you want to delete template(s): __________?". Click the Yes button to delete the template. |
| 5. | To create a new template, complete the following steps: |
| a. | Click the Create Template button.
RESULT: The Template Details page appears. |
| b. | Enter the following details to be saved in this template: |
| i. | Template Name. This is a required field. |
| ii. | Select a Consignee from the drop-down list. |
| 1) | To add a new consignee, click the Add button.
RESULT: The Consignee Details page appears. |
| 2) | Enter the required information for a consignee.
NOTE: If the Consignee Alias Name, which is the consignee name represented in a drop-down, is left empty, the first 40 characters from Consignee Name will be copied to it on save. |
| 3) | Click either the Save & Back or the Save & Add Another button to continue. |
| 1) | In the Merchandise field, click the Details button.
RESULT: The Template Merchandise page appears. |
| 2) | To add rows for each article or component of an article, enter the HITSUS Number and Article/Component and click the Enter Row Details button to select the scientific name and enter row specific data.
RESULT: The Template Merchandise Details page appears. |
| 3) | Enter the scientific name for the article/component. If the species of plant used to produce the product varies and the species used to produce the product is unknown, enter each species that may have been used to produce the product. |
| 4) | Enter a common name or a botanical name to search for a commodity. From the Commodity search results, enter the number of rows to be added to the form for each commodity and click Generate Rows. The corresponding Scientific name (botanical name) will be added to the List of Articles. Any search will return a maximum of 1000 results. Or if the product was manufactured prior to the Amendment and you cannot determine species, you can select the applicable Special Use option.
NOTE: If no rows have been added and you click the Save button, the Article/Component of Article will not be added to the template. If you are editing an existing Article/Component of Article and you delete all the rows, the Article/Component of Article will be removed from this template once you click the Save button. |
| 5) | If you need to add more rows after returning from adding row details, enter the additional HITSUS Number and Article/Component and click the Enter Row Details button. After saving the row details the new rows will appear at the end of the existing table. Previously entered data will not be lost. |
| 6. | Select Save to save the information and return to the list of templates. |
